Age restrictions
- Tattoos: ages 16–17 only, with a notarized parental consent form and a parent or legal guardian present. No tattoos under 16 — Florida law, no exceptions.
- Piercings: under 18 requires guardian consent. Under 16, the guardian must remain present for the entire procedure.
Required documentation
- Parent or legal guardian's government-issued photo ID.
- Minor's photo ID or birth certificate.
- Completed notarized consent form.
- Proof of guardianship if last names differ.
