Minor Consent

Bringing your teen in? Read this first.

Tattoos and piercings for minors have strict legal requirements. Here's exactly what's allowed, what to bring, and how to complete the pre-check ahead of time so check-in is quick.

This form is a pre-check only. Final eligibility, ID review, parent/legal guardian verification, notarized consent, and in-studio approval must be completed according to studio policy and applicable requirements.

Age restrictions

  • Tattoos: ages 16–17 only, with a notarized parental consent form and a parent or legal guardian present. No tattoos under 16 — Florida law, no exceptions.
  • Piercings: under 18 requires guardian consent. Under 16, the guardian must remain present for the entire procedure.

Required documentation

  • Parent or legal guardian's government-issued photo ID.
  • Minor's photo ID or birth certificate.
  • Completed notarized consent form.
  • Proof of guardianship if last names differ.
